Are you aged 12-24 & living in the Black Country?

We are asking children and young people aged 12-24 from Dudley, Sandwell, Walsall, and Wolverhampton their views
about how mental health services can encourage people to get involved in helping to improve these services.

The survey will take approximately 15 minutes and will be completely anonymous.
